Abstract

In the originally published version of this article, the ERACODA collaborators were incorrectly listed. The names and affiliations of these Collaborating authors, and the number of records entered in the ERACODA database have now beenmoved from the accompanying supplementary material and are listed as a subsection within the Acknowledgements. The pagination of the article has been adjusted to reflect this change. Supplementary Figures S1 and S2 were inadvertently omitted. These have been reinstated as part of the Supplementary Data.
