A 69-year-old man with history of a heart transplant 6 months earlier for ischemic cardiomyopathy presented with 2 days of left-sided, sharp, pleuritic chest pain associated with subjective fevers and chills. He complained of a cough productive of white sputum but denied sore throat or sinus congestion. He had no sick contacts and no recent travel.
