Though initially controversial, liver transplantation (LT) currently plays a vital role in definitive treatment for alcoholic cirrhosis, and is now one of the most common indications. Data are emerging that LT for severe alcoholic hepatitis that is unresponsive to medical management may also play a role, though it is more controversial. The main concern in alcoholic patients is risk of relapse, and post-transplant interventions have been shown to reduce recidivism. Special considerations for patients transplanted for alcoholic liver disease center around post-transplant interventions to improve mortality, such as tobacco cessation and surveillance for malignancy.
