This 35-year-old lady sustained a whiplash-type injury in a motor vehicle accident 1 month prior to assessment. Since then she has been complaining of neck pain. Physical examination revealed painful limitation of neck movement and a normal neurological examination. Flexion and extension lateral X-rays and MRI of the cervical spine confirmed the diagnosis of long-standing atlantoaxial instability (Fig. 9.1a-c).
