Excisional hemorrhoidectomy remains the surgical approach to symptomatic hemorrhoids that are not suitable for less invasive forms of surgical or office procedures, and failed nonoperative management. This chapter describes both local anesthesia for minor anorectal procedures and standard techniques for conventional surgical hemorrhoidectomy. Indications, pitfalls and danger points, preoperative preparation, operative strategy and technique, postoperative care, and complications are all discussed.
