PT Sakai Indonesia is a heavy equipment manufacturer for road construction which is a subsidiary of Sakai Heavy Industry in Japan which is committed to making continuous improvements in various aspects. One way is to control product quality. In its production activities, PT Sakai Indonesia does not fully produce products that meet specifications, the portion of NG produced by production activities reaches 20% of all production activities which exceeds the maximum target set by the company which is 10%. The analysis methods used in this research are Six Sigma and FMEA. The sample in this study is production defect data for the period January - September 2023. The data is processed using Ms.Excel as well as direct observation and interviews. Through this analysis, the sigma value of 3.29 and the highest RPN of 140 were obtained, the main factor causing product defects can be known, namely there is no SOP in the production process. For this reason, it is necessary to make SOPs in each production process in order to minimize product defects.
