BACKGROUNDThe age of onset of rheumatoid arthritis (RA) appears to be a determining factor in differences in disease presentation.OBJECTIVETo study the clinical, demographic and treatment characteristics of patients with elderly-onset rheumatoid arthritis (EORA) and to compare them to those with younger-onset rheumatoid arthritis (YORA) patients.METHODSWe studied 62 patients with EORA and 111 with YORA for gender, presence of rheumatoid nodules, interstitial pneumonitis, tobacco exposure, rheumatoid factor (RF) and antinuclear antibodies (ANA), medications, functional index measured by Health Assessment Questionnaire (HAQ) and inflammatory activity measured by DAS (Disease Activity Score) 28.RESULTSHigher prevalence of male gender (p=0.02), positive RF (p=0.007) and lower HAQ scores (p=0.04) were found in EORA's patients. There were no differences regarding the use of medications, presence of nodules and interstitial pneumonitis, ANA and tobacco exposure (p=NS).CONCLUSIONIndividuals with EORA have a higher prevalence of male gender and RF positivity and less functional impairment than individuals with YORA.JUSTIFICATIVAA idade de início da artrite reumatoide (AR) parece ser um fator determinante em diferenças na apresentação da doença.OBJETIVOEstudar as características clínicas, demográficas e de tratamento dos pacientes com EORA (elderly onset rheumatoid arthritis ou com início ≥ 60 anos de idade) em comparação com os de YORA (young onset rheumatoid arthritis) pareados para tempo de duração de doença.MÉTODOSEstudaram-se 62 indivíduos com EORA e 111 com YORA para gênero, presença de nódulos reumatóides, pneumonite intersticial, exposição ao fumo, fator reumatoide (FR) e fator antinuclear (FAN), uso de medicamentos, índice funcional medido pelo HAQ (Health Assessment Questionnaire) e atividade inflamatória medida pelo DAS (Disease Activity Score) 28.RESULTADOSObservou-se que no grupo de pacientes com EORA existia uma maior prevalência de homens (p=0,02), de FR positivo (p=0,007) e menor pontuação no HAQ (p=0,04). Não foram encontradas diferenças quanto ao uso de medicamentos, presença de nódulos e pneumonite intersticial, FAN e exposição a fumo (p=NS).CONCLUSÃONa EORA existe maior prevalência do sexo masculino e de positividade para o FR e menor prejuízo funcional do que em YORA.
