Ever since the first description of moyamoya in 1957, many advances in diagnosis and treatment have been made. While the treatment of the disease mainly remains surgical due to lack of medical treatment options, the general clinical management as well as pre-, peri-, and postoperative care is of utmost importance in order to minimize the ramifications of the disease. In this chapter we try to depict the characteristics of different forms of moyamoya; discuss regional differences; give an overview of current methods in diagnosis, conservative, as well as surgical treatment; and illustrate crucial steps in the clinical management of moyamoya.
