Abstract

Pyloroplasty is now primarily used in patients undergoing emergency surgery for massive hemorrhage from duodenal ulcer when other methods of control have failed. A truncal vagotomy may be added if the patient has been noncompliant with medical therapy (see Chaps. 25 and 26). This chapter describes several techniques for pyloroplasty and considerations for suture ligation of the gastroduodenal artery. Indications, preoperative preparation, pitfalls, surgical strategy and technique, and complications are all discussed.
