Integrative Care Planning

Abstract

Within mental health care, various forms of care plans exist, including recovery care planning, joint crisis planning, discharge planning, and advanced mental health directives. Different principles, guidelines, algorithms, and consensus guidelines for these forms of care planning and treatment pathways exist, but generally these guidelines do not consider a holistic, person-centered approach that is central to mental health nursing. In this chapter focus is placed on the importance of shared decision-making which lies at the heart of collaborative care planning, where the voice and will of the service user is paramount. Distinct types of care plans are discussed, with emphasis on service user and carer participation. To facilitate understanding of the clinical relevance of integrative mental health nursing and collaborative practice for advanced practice mental health nurses (APMHNs), this chapter includes the application of an integrative care plan to a person facing mental health challenges.
