Abstract

The objective of this study was to describe the translation and adaptation of the questionnaire used in the Primary Care International Study of Medical Errors (PCISME) for application in primary health care in a local health district in Rio de Janeiro State, Brazil. The process included organization of an expert panel that adapted the questionnaire using a modified Delphi method. The adapted instrument can help produce specific information on primary health care and strengthen initiatives to improve patient safety in Brazil.
