Abstract

Transitional care is defined as the interventions that are provided in the process of relocating the patient from hospital to home. Faith community nurses can be intricately involved in transitional care. Research that defines transitional care in faith community nursing is reviewed with discussion related to the difference in outcomes when the faith community nurse assumes the responsibility for the transitional care of the patient. Ongoing research is needed to continue to document the contribution that faith community nurses can make to reforming the delivery of care in the community.
