There are numerous functional and anatomical disturbances that can interfere with male sexuality. Purely mechanical disturbances of semen deposition due to anatomical features have to be differentiated from functional disturbances of libido and orgasm, from ejaculation disorders and the inability to achieve or maintain a rigid erection which allows penetration. While in the vast majority of cases disorders of orgasm are of psychological origin, disturbances of libido may be due to hormonal imbalance and can be a symptom of androgen deficiency. Further symptoms such as alterations of the secondary sexual characteristics or of semen volume point to organic origin. While functional disturbances of orgasm and libido fall into the realm of psychological and psychiatric or endocrinological therapy, this chapter deals mainly with disturbances of semen deposition and functional or organic impairment of erection.
