Mid-term results of ceramic-on-ceramic bearing extensively porous coated AML® total hip arthroplasty

Abstract

Purpose: This study evaluated the mid term results of primary total hip arthroplasty using a ceramic-on-ceramic bearing extensively porous coated implant. Materials and Methods: Between April 2001 and August 2004, THA using a ceramic on ceramic bearing cementless implant was performed on 140 cases (131 patients). Among these cases, 138 hips form 129 patients, who were followed up for more than three years were evaluated. The average age was 51.2 years (19-84). The initial diagnosis included osteonecrosis of the femoral head in 87 cases, primary osteoarthritis in 9 cases, pyogenic arthritis sequelae in 9 cases, traumatic osteoarthritis in 8 cases, acetabular dysplasia in 7 cases, femoral neck fractures in 6 cases, an ankylosed hip in 5 cases, and others diagnoses in 7 cases. Results: On the last X-ray, there was no sign of loosening of the acetabular cup. However, one femoral stem was definitely loose and four femoral stems were possibly loose. The other femoral stems were fixed well by bone ingrowth, and there was no case of osteolysis. Chip fracture of the ceramic liner occurred intraoperatively in 2 cases. Posterior dislocation occurred 3 weeks after surgery in one case, anterior dislocation with a ceramic liner dissociation occurred 6 weeks after surgery in one case and fracture of the ceramic head occurred 15 months after surgery in one case. Conclusion: The mid-term results of ceramic-on-ceramic bearing extensively porous coated implant were reasonable. However, a long term follow up will be needed to determine the overall results.
