An interesting case of spontaneous appendico-cutaneous fistula, probably the first case to be reported from Pakistan is being presented. A 16 years old female presented with a discharging sinus in the right iliac fossa since last eight months. Fistulogram showed its connection to ileo-caecal region. At laparotomy the fistula tract was found to be appendix itself. Appendicectomy was done. Histopathology revealed chronic non-specific inflammation.
