Observational studies have shown a strong association between an energy-protein deficit and increased morbidity and mortality in critically ill patients. However, recent prospective, randomized studies examining nutritional interventions have often given divergent results, which may lead to confusion. The TICACOS study attempted to elucidate the optimal method for both defining an energy target using indirect calorimetry and delivering the measured nutritional support. The clinical outcomes of this study as well as those of other recent studies are discussed. Practical suggestions based on best available evidence to assist the practitioner in prescribing nutritional support are also presented.
