Abstract

Background: Approximately 30 % of all cervical spine injuries affect the atlas and axis. Fractures predominantly occur in elderly patients, while younger patients are more likely to have ligamentous disruptions. As the anatomy and therefore the pathophysiology differ from the other cervical vertebrae, a separate classification and special treatment guidelines have to be considered. Treatment: Unstable fractures of the atlas and dens axis must be treated operatively. Direct monovertebral osteosynthesis and spondylodesis are the standard types of surgery. Selecting the appropriate procedure is paramount for fracture healing.
