Önemsenmiyor mu, bilgi eksikliği mi ?: ramak kala olaylar

Abstract

Bir olayın gerçekleşmeden önce belirlenmesi, alınacak önlemlerle o olayın gelecekte oluşturacağı zararın önlenmesine belirgin katkı sunmaktadır. Bu nedenle ramak kala olayların tespiti, istenmeyen olayların gerçekleşme potansiyelini düşürmesi açısından önemlidir. Bu çalışmada, hazırlanan bir anket aracılığıyla hastanemiz çalışanlarının ramak kala olaylarla ilgili bilgi düzeyini değerlendirmek, farkındalıklarını arttırmak ve son 3 ay içinde karşılaştıkları ya da gözlemledikleri ramak kala olayı bildirmelerini sağlamak amaçlandı. Anketin örneklemi, hasta bakım ve hizmet süreçlerinde doğrudan görev alan meslek gruplarından 50 ve üzerinde personele sahip olanların en az %20'si olacak şekilde belirlendi ve toplam 290 kişiye anket uygulandı. Katılımcıların % 96,90’ının ramak kala olayın ne olduğunu bildikleri, % 91,03’ünün hasta güvenliğiyle ilgili, %81,72’sinin çalışan güvenliğiyle ilgili ramak kala olay örneğini doğru tanımladıkları gözlendi. Katılımcıların % 16,90’ı son 3 ay içinde hasta ya da çalışan güvenliğiyle ilgili bir ramak kala olay yaşadıklarını/gözlemlediklerini ifade ettiler. Bu olayların %69,77’si bildirilmemişti. Bildirmeme gerekçesini vakti olmadığı şeklinde ifade edenlerin oranı %26,66 ve bildirim yapılacağını bilmediğini söyleyenlerin oranı %23,33 olarak gözlendi. Sonuç olarak, hasta ve çalışan güvenliğiyle ilgili ramak kala olay bildirimlerinin arttırılmasını sağlayacak çalışmaların planlanması, hasta ve çalışanlar için daha güvenli hastaneler oluşturulmasına katkıda bulunacaktır.Noticing of an incident before it happens helps to prevent potential damages due to the incident by taking precautions. Therefore, the determination of incidents is significant in terms of reduction of realization possibility of unintended incidents. In this study, it was aimed to evaluate the knowledge level and increase the awareness level of our hospital staff, and to make them report the incidents they had encountered or observed in the last 3 months with the help of a questionnaire.The sampling of the survey was determined to be at least 20% of the professional groups directly involved in the patient care and service processes that have 50 or more staff and the survey was applied to a total of 290 staff. When the survey results were evaluated; it was observed that 96,90 % of the participants knew what the near miss incident was, 91,03 % of the participants defined correctly the near miss incident case related to the safety of patients and 81,72 % of the participants defined correctly the near miss incident case related to the safety of staff. 16, 90% of participants stated that they have observed a near miss incident related to safety of patients or staff during the last 3 months. However, 69, 77 percent of these incidents had not been reported. It was observed that the ratio of the participants who stated that they hadn’t had any time for reporting was 26,66% and who stated that they hadn’t known that it was mandatory to report was 23,33%. As a result, the planning of studies to ensure the reporting of incidents related to both staff and patient safety will contribute to the establishment of safer hospitals for patients and staff.
