A 3-year-old girl evaluated because of a cardiac murmur and low weight gain. A large secundum atrial septal defect (ASD OS) was diagnosed, and percutaneous closure of the defect was performed. The procedure was complicated by left ventricular migration of the device, leading to urgent cardiac surgery.
