Abstract

Aim: In the SEKOIA study, strontium ranelate (SrRan) demonstrated radiographically a structure-modifying activity in knee osteoarthritis. The aim of this analysis was to evaluate the effect of SrRan on knee cartilage and bone in a subset of patients that underwent MRI. Method(s): SEKOIA included patients over 50 years old with symptomatic primary knee osteoarthritis. MRI was performed at inclusion and annually. Images were centrally read and main joint components (cartilage morphology, bone oedema, subarticular cysts, subchondral bone attrition and osteophytes, meniscal damage, synovial effusion) were evaluated using the WORMS score. SrRan 1 g/day and SrRan 2 g/day treatment groups were compared to placebo on the change from baseline to last value using a general linear model adjusted with gender, centre and baseline as covariates. Result(s): MRI ITT included 364 patients. Over 3 years, progression in WORMS score occurred in less than 45% of patients for cartilage morphology and less than 20% of patients for other joint features.The number of patients with a deterioration of the cartilage morphology was significantly lower in the SrRan 2 g group than in the placebo group in the lateral compartment (3.4%, vs 15.7%, p = 0.001). Fewer patients had a worsening of their osteophytes score in the medial compartment with SrRan 2 g than in the placebo group (12.0% vs 23.0%, p = 0.026). No statistically significant differences were observed for the SrRan 1 g group. Despite the small number of patients concerned, numerically fewer patients had a worsening of bone attrition score in the SrRan 2 g group than placebo (2.6% vs 6.6%) in the medial compartment. Conclusion(s):Within the limits of the small number of patients with detectable progression, strontium ranelate 2 g/day tended to reduced cartilage degradation and progression of bone osteophytes in patients with knee osteoarthritis.
