Abstract

Health systems globally are struggling to respond to a rapidly ageing popula-tion and promote positive trajectories for healthy ageing. Integrated care models provide an avenue towards care being delivered in a coordinated and structured manner across different levels of care (primary, secondary and tertiary). Tech-nology is vital in providing timely and effective integrated care for older people. Emerging technologies can play a role both in aged care facilities, as well as in-home services. This chapter discusses the role of technology in facilitating or enhancing integrated care models in aged care.
