In summary, a young female patient with probable fibromuscular renovascular hypertension with a persistent high renin state, lateralizing captopril renal scintigraphy, and a positive MRA for renal artery occlusion is presented. Following a period of sustained blood pressure control on three-drug medical therapy, accelerated stage 3 hypertension was attributed to the lack of an ACE inhibitor. Excellent long-term control was achieved when a low-dose ACE inhibitor was added to a thiazide diuretic.
