Emergent splenectomy is typically done in the context of an exploratory laparotomy for trauma. The role of splenorrhaphy is limited to the stable trauma patient who has no other major injury. The majority of patients who suffer major splenic injury requiring laparotomy are best managed with a splenectomy. Today, most patients with splenic injury are managed nonoperatively. This chapter describes the approach to splenic trauma. It includes indications, preparation, pitfalls, operative strategy and technique, and postoperative care.
