Chronic diseases, such as heart disease, stroke, chronic respiratory disease and diabetes are the major cause of death in the world. This paper aims at presenting a low cost, secure medical platform for mobile and desktop operating systems to monitor vital parameters, receive notifications when the pre-defined limits are out of range and foresee a patient’s health status. This system processes medical data effectively by correlating the parameter trends with the patient’s actual state to anticipate their health evolution. The purpose of the platform is to offer up-to-date information about chronic diseases in order to permanently ensure patients’ safety and adequate treatment. In a systemic approach, this chapter is a complement to a publication presented in the workshop of the 8th International Conference on Pervasive Computing Tech- nologies for Healthcare (PervasiveHealth ‘14) [1]. Taking into consideration the previous work, we present a concrete scenario of how the system works, describe the system components, and discuss some aspects regarding energy consumption and autonomy.
