Home care nurses frequently manage patients who also receive a substantial amount of care from a lay caregiver in the home. Because many of these care givers are themselves frail and elderly, the home care nurse must assist both the patient and the caregiver to cope with demands for physical care, complex technology, and behavioral manifestations of chronic conditions. Specialty practice nurses are in an ideal position to assist both the primary visiting nurse and the lay caregiver in the management of complex wound, ostomy, and continence care associated with early discharge from the hospital.
