Obtaining vascular access can be difficult following lead extraction if a wire is not retained within the vessel. We describe a unique situation in which retrograde passage of a guidewire to the explanted internal cardioverter/ defibrillator pocket facilitated regaining subclavian venous access. © 2006, The Authors.
