The article describes the perioperative course of the longest separated survivor of cardiopagus twins joined at both atria and ventricles. The initial success of this procedure is attributed to clear communication between members of the separation team, accurate preoperative understanding of the cardiovascular anatomy, and careful iv blood and fluid replacement.
