We report on 26 sportsmen with chronic knee instability due to anterior cruciate ligament (ACL) insufficiency combined with Grade II–III medial lesions in whom ACL reconstruction was associated with a mini-invasive medial ligament plasty. At a mean follow-up of 38 months, 24 out of 26 patients had returned to sport at the preinjury level. Knee function was restored to normal or nearly normal in all cases without any operative complication. Medial stability can be regained with an anatomic repair without pes plasty. The medial side should not be overlooked in chronic combined anteromedial instabilities.
