Brachymetatarsia is a condition that occurs when the epiphyseal growth plate in one or more metatarsals closes prematurely, causing anatomic shortening. Callus distraction with the use of a miniature external fixator is employed for deficits greater than 1 cm in length. Preoperative clinical and radiographic analyses are critical to successful operative correction and entail both weightbearing and non-weightbearing exams as well as complete medical evaluation to assess for concomitant medical conditions. A surgical technique is outlined with a modification to decrease postoperative stiffness at the metatarsophalangeal joint, which is a common complication of callus distraction. Other complications are discussed and treatment options outlined.
