Introduction: Hospital malnutrition affects 30% to 50% of hospitalized patients, contributing to increased complications, length of stay, readmission, mortality and hospital costs. The aim of this study was, inspired by the precepts of the “Campaign say no to malnutrition”, to audit actions to diagnose and treated malnutrition in hospitalized patients. Methods: Retrospective study with adult hospitalized patients for clinical and surgical treatment at a University Hospital in Cuiabá-MT. The end point was the notification of the nutritional status in the first 24 to 48 hours of hospitalization and the nutritional conduct prescribed for patients according to nutritional status. Results: Among 1595 patients, 55.5% (n= 886) were female, with a median age of 49 years, ranging 36 to 62 years. Within 24-48 hours of admission, 1030 (64.6%) patients were screened by NRS-2002 and 90% (n = 927) were at nutritional risk. Among patients at nutritional risk, 924 (99.7%) underwent nutritional assessment by SGA and 70.7% (n = 654) had malnutrition (SGA= B or C). Thus, 924 (57.9%) patients were effectively screened by NRS-2002 and evaluated by ASG. Of the 927 patients at risk of malnutrition, 189 (20.4%) received ONS and 76 (8.2%) NT. ONS and TN were prescribed for 187 (28.6%) and 69 (10.5%) malnourished patients, respectively (SGA = B or C). Conclusions: About 60% of the patients underwent a routine nutritional assessment, however, less than 30% of patients at nutritional risk or malnourished, received any nutritional intervention by ONS or NT.
