Abstract

Introduction: The TEP approach could be the closest to ideal option to repair inguinal hernias but has limitations such as a reduced surgical field and a restricted port set up. To overcome these problems, the extended modality (eTEP) has been described, which creates a larger surgical field and allows greater flexibility in the positioning of the working ports. The objective of this manuscript is to review this modality of approach as an alternative to treat inguinal hernias. Methods: Systematic review based on the Cochrane, Google, Embase and Medline databases using PubMed, without restrictions and using as a keyword: totally extraperitoneal technique. Results: The current experience is described with the extended approach and its technical steps for a correct creation of the space and subsequent placement of trocars. Conclusion: Our initial multicenter evaluation of the eTEP access technique for inguinal hernias has found the approach feasible and effective. This novel approach offers flexible port set-up optimal for laparoscopic inguinal hernias.
