P589Prognostic value of systemic immune-inflammation index in patients with chronic heart failure

Abstract

Background: Recently, systemic inflammation plays a key role in the prognosis of chronic heart failure (CHF) patients. Systemic immune-inflammation index (SII), which is calculated by the formula: neutrophil x platelet/lymphocyte, is a useful prognostic marker in several types of cancer. However, there is no information available regarding the prognostic value of SII in CHF patients. Methods and Results: We studied 104 CHF outpatients with LVEF <40% in our prospective cohort study. We measured laboratory data and echocardiography at entry. During a follow up period of 6.3+/-4.5 years, 51 patients had cardiac events, defined as readmission for worsening heart failure or cardiac death. At multivariate Cox analysis, SII was significantly associated with cardiac event independently of serum sodium and uric acid levels, and LVEF after the adjustment of serum creatinine level. ROC analysis revealed that SII of 551 was a fair discrimi- nator for cardiac event (AUC 0.723 [95% CI 0.627-0.807]). Kaplan-Meier analysis revealed that patients with high SII (>551) had a significantly greater risk of cardiac event (85% vs 36% p<0.0001, adjusted HR 3.34 [1.92-5.83]). Conclusion(s): Systemic immune-inflammation index could provide the improved prediction of poor outcome in CHF patients. (Figure Presented).
