Abstract

Zusammenfassung Durch den hohen Funktionsanspruch kommt der Rekonstruktion von Rotatorenmanschettenrupturen eine große Bedeutung zu.Basis für eine optimale Therapie sind neben der klinischen und radiologischen Untersuchung der Funktionsanspruch und die Motivation des Patienten. Ziele sind die Wiederherstellung der Schmerzfreiheit und die Wiedererlangung der Funktion.Bei traumatischen Rupturen sind eine frühzeitige Naht bzw.Rekonstruktion anzustreben.Für die operative Therapie von Rotatorenmanschettenrupturen werden zahlreiche offene und arthroskopische Operationsverfahren empfohlen.Bei nicht rekonstruierbaren Rupturen stehen Ersatzverfahren zur Verfügung, die eine funktionelle Rekonstruktion ermöglichen. Besteht ein inoperabler Defekt, bieten palliative Maßnahmen oder bei begleitender Omarthrose endoprothetische Verfahren wie die inverse Prothese, eine gute Alternative. Bei richtiger Indikationsstellung und präziser Operationstechnik können gute Langzeitresultate erreicht werden. Abstract Reconstruction of the rotator cuff is extremely important in the case of a tear, because of the high functional demands on this structure. Clinical and radiological criteria and the patient's expectations of a functional shoulder joint and his or her motivation combine to form the basis on which the optimal therapy is selected.The goal of the treatment is freedom from pain and the restoration of shoulder function. An early surgical intervention, in the form of either suturing or reconstruction, is necessary in the case of traumatic ruptures.A number of open and arthroscopic techniques are recommended for rotator cuff repair. In cases of irreparable rotator cuff defects various other surgical options can be considered.Biceps tenotomy can be performed as a palliative procedure.The inverse total shoulder prosthesis will probably be the treatment of choice in cases of irreparable rotator cuff defects with painful arthropathy.When the correct indications are observed and a precise operating technique is applied it is possible to achieve good long-term results.
