Abstract

Complex computer systems are rarely developed on time, within budget, and up to spec. In the paper are just a few examples from the RISKS archives. Among the case study presented are: Virginia system for distributing child support checks, Bank of America computerized trust accounting and reporting system, Allstate Insurance computerized automation system, Blue Cross and Blue Shield of Wisconsin with an Electronic Data System (EDS) that did not worked. Other systems discussed are the one used for the U.S. Office of surface Mining to prevent violators of strip-mine laws from getting new permits, and the Airborne Self-Protection Jammer (ASJP), an electronic air-defense system installed in over 2,00 Navy fighters and attack planes.
