Scientific and popular interest in various ketogenic interventions has increased in recent years. Multiple studies have reported positive results for people living with type 2 diabetes ranging from allowing medication tapering to the complete remission of the disease. However, many misconceptions and conceptual errors persist among health professionals and even in the scientific literature. It is often assumed that the different ketogenic interventions are interchangeable and many physiological concepts that are relevant for clinical management or biomedical research are frequently ignored or overlooked. This text sets out the physiological bases that support the use of ketogenic interventions as part of the treatment of type 2 diabetes mellitus and describes the areas of uncertainty in order to guide future biomedical research projects.
