Asthma has a complex aetiology with many precipitating factors leading to a final common pathway of airway inflammation and bronchospasm. Patients with asthma are at risk of developing acute exacerbations. A subgroup of these patients develop severe exacerbations. This review will focus on some of the issues in the management of adults with life threatening asthma that remain controversial.
