Integrating mHealth into Primary Care: A Stakeholder-Informed and Systems Framework

Abstract

Background: Mobile health (mHealth) applications offer promising tools to extend behavioral health care within Integrated Primary Care (IPC). However, the implementation of mHealth innovations into routine IPC practice is hindered by patient-, provider-, app-, and system-level challenges. Purpose: This commentary proposes a conceptual framework to guide the successful integration of mHealth applications into IPC settings by identifying key factors across stakeholder and system domains. Building on established models (CFIR, RE-AIM, TAM), this framework translates implementation principles into four interdependent domains—patient, provider, app, and system—tailored to Integrated Primary Care. Methods: Using a translational lens, this commentary synthesized literature, clinical experience, and implementation considerations to develop a four-domain framework encompassing patient, provider, app, and system-level factors. The domains were derived through a narrative synthesis of established implementation science frameworks—including CFIR, RE-AIM, and the Technology Acceptance Model (TAM). No empirical data were collected for this conceptual work, and institutional review board (IRB) review was therefore not applicable. The framework was developed through a narrative synthesis of the existing literature and the author’s applied experience working within Integrated Primary Care teams. Insights from implementation science models were integrated with practical observations from clinical practice to ensure conceptual coherence and real-world relevance. Results: The proposed framework identifies digital and health literacy, provider confidence in evidence-based tools, app usability and credibility, and system-level readiness (e.g., EHR integration, governance, reimbursement) as critical factors. Interactions between these domains create opportunities for reinforcing engagement and reducing implementation barriers. Conclusions: The conceptual framework offers practical guidance for clinicians, researchers, and policymakers working to advance the integration of mHealth into IPC. Future work should apply this framework in implementation research and policy design to enhance behavioral health care delivery and equity through digital tools. By bridging research and real-world implementation, this framework supports scalable strategies to enhance behavioral health equity across primary care settings.
