Abstract

In the originally published version of this manuscript, the affiliation of Chris Longhurst was incorrect. Dr Longhurst's affiliation should read, "Department of Medicine, UC San Diego Health, La Jolla, California, USA", instead of, "Center for Clinical Informatics and Improvement Research, University of California San Francisco, San Francisco, California, USA." This error has been corrected.
