MODELO DE ATENCIÓN CENTRADO EN LA PERSONA CON MULTIMORBILIDAD (MACEP)

Abstract

Multimorbidity is a national and global public health problem that causes a deterioration in people’s health and survival. Health system response is fragmented in several programs with low satisfaction and adherence in patients. Therefore, is advisable to move towards a strategy applying the principles of the Model of Comprehensive Community Health, where the focus ison people, who are activated for their health care. The Ancora Health Innovation Center of the “Universidad Católica” and the Metropolitan Southeast Health Service (SSMSO) carried out a pilot project to implement the Person-Centered Care Model with Multimorbidity (MACEP) aiming to prevent complications and reducing demand at secondary and tertiary care levels. Between 2017 and 2019, 20,642 people were intervened and to evaluate intervention impact they were compared with 32,661 people treated in neighboring not intervened Family Health Care Centers (CESFAM). Significant decreases in the risk of dying and hospitalization were found in the intervened population. Percentage level of MACEP implementation in the seven intervened family health care centers was also evaluated,verifying values from 41 to 100%. The economic evaluation of the pilot project showed higher costs in primary and secondary care, as well as significant savings at the tertiary level, witha net saving in the system total cost. In 2019, the Ministry of Health,drawing on MACEP's experience in the SSMSO, described the Strategy for Person-Centered Comprehensive Care in the Context of Multimorbidity (ECICEP) and began its implementation at the national level. In the case of a public policy that it is still very young,it becomes necessary to deepen the changes that ensure its progress and consolidation.La multimorbilidad constituye un problema de salud pública nacional y mundial que provoca un menoscabo en la salud y la sobrevida de las personas. La respuesta del sistema de atención está fragmentada en torno a programas, determinando baja satisfacción y adherencia en los pacientes. Se requiere avanzar hacia una estrategia que aplique los principios del Modelo de Atención Integral en Salud Familiar y Comunitaria, donde el foco esté centrado en las personas, quienes se activan para el cuidado de su salud. El Centro de Innovación en Salud Ancora de la Universidad Católica, junto al Servicio de Salud Metropolitano Sur Oriente (SSMSO), realizó un proyecto piloto de implementación del Modelo de Atención Centrado en la Persona con Multimorbilidad (MACEP), con el objetivo de prevenir complicaciones y disminuir demanda en los niveles secundarios y terciarios de atención. Entre los años 2017 y 2019 se intervinieron 20.642 personas con el MACEP. Para evaluar el impacto, esta población se comparó con 32.881 personas atendidas en Centros de Salud Familiar (CESFAM) vecinos no intervenidos, demos­trándose una disminución significativa en el riesgo de morir y de ser hospitalizado en la pobla­ción intervenida. Se evaluó, además, el nivel porcentual de implementación del MACEP en los siete centros de salud familiar intervenidos, verificando valores entre 41 y 100%. La evaluación económica del proyecto piloto mostró un mayor costo en atención primaria y secundaria, así como un ahorro significativo en el nivel terciario, lo que, finalmente, genera un ahorro neto en el costo global del sistema. El año 2019 el Ministerio de Salud, aprovechando la experiencia del MACEP en el SSMSO, describió la Estrategia de Cuidado Integral Centrada en la Persona en contexto de multimorbilidad (ECICEP) e inició su implementación a nivel nacional. Tratándose de una política pública aún muy joven, es necesario profundizar en los cambios que aseguren su progreso y consolidación.
