Abstract

A man in heart failure with a high oxygen affinity haemoglobin variant (Hb Rainier) underwent a mitral comrnissurotomy with the aid of cardiopulmonary bypass. Pre‐operatively, a total blood exchange transfusion was carried out to prevent potential hypoxic and thrombo‐embolic complications. No complications occurred in the postoperative period.
