Abstract
Background: In recent years, the implementation of extended therapeutic measures by paramedics based on standard operating procedures (SOP) has increased in emergency medical services (EMS). However, research concerning the quality of processes and results in Germany is rare. Objectives: The subject of the publication is the evaluation of the extended measures to ensure a high level of quality and safety, as well as the comparison with other working groups. Furthermore, this is intended to encourage other areas of EMS to establish similar systematic processes and promote the implementation of the requirements of the German Emergency Paramedic Act (NotSanG). Materials and methods: Systematic documentation and statistical evaluation of extended care measures in the EMS cooperation in Schleswig-Holstein (Rettungsdienst-Kooperation in Schleswig-Holstein, RKiSH) from 01 January 2015 to 31 December 2019 with regard to indication, implementation, documentation, complications, and underlying emergency situations. Results: Extended procedures were documented in less than 2% of all rescue missions. Indication was deemed adequate in 98.4%. Conclusions: The number of extended measures in our sample corresponds to the results of similar investigations. Deficits in standardized documentation were identified in 29% and may endanger patient safety as well as lead to liability risks.
