Association of CRP Gene Polymorphism and CRP Levels in Asian Indians With OSA and NAFLD

Abstract

PURPOSE: The aim of the present study was to investigate the association of CRP gene [SNP +1444 C>T (rs1130864)] and CRP levels in OSA and NAFLD. METHODS: 180 subjects 74 OSA with NAFLD (group 1), 35 OSA without NAFLD (group 2), 42 without OSA and with NAFLD (group 3) and 29 without OSA and without NAFLD (group 4) were evaluated. Degree of NAFLD was based on abdomen liver ultrasound and of OSA on overnight polysomnography. Clinical, anthropometric, body composition and biochemical levels was measured in all subjects. Fasting serum insulin and CRP levels were determined. PCR-RFLP of CRP gene was performed. RESULTS: Blood pressure [systolic (p=0.001) and diastolic (p=0.0003], anthropometric parameters [waist circumference (p=0.001), hip circumference (p=003), subscapular (p=0.003), lateral thoracic (p=0.002) and suprailiac (p=0.002)], body composition [%body fat (p=0.003) and body fat (kg) (p=0.01) and biochemical parameters [fasting blood glucose (p=0.05), serum triglyceride (p=0.001), total cholesterol (p=0.002), high density lipoprotein-cholesterol (p=0.05) and alanine aminotransferase (p=0.01), CRP (P=0.002), serum insulin (p=0.05) and HOMA-IR (p=0.04)] were significantly higher in OSA and NAFLD group as compared to other groups. Serum CRP levels were significantly higher in group 1, group 2 and group 3 as compared with healthy obese subjects (p=0.002). The SNP rs1130864 TT genotype was more prevalent in individuals with OSA and NAFLD and obese (p=0.03), as a consequence frequency of the minor allele T was significantly higher in group 1 (p=0.003). The TT genotype of SNP rs1130864 was associated with increased body mass index (p=0.001), total cholesterol (p=0.002), serum triglyceride (p=0.003), alkaline phosphatase (p=0.04), waist circumference (p=0.02), fasting insulin (p=0.003) and HOMA-IR (p=0.02) in OSA and NAFLD subjects as compared to other three groups. The SNP rs1130864 TT genotype has been associated with increased levels of CRP (p = 0.002). Using a multivariate logistic regression model, subjects with TT genotype showed higher risk of OSA and NAFLD (OR, 4.52, 95% CI: 3.16-9.56, p=0.002). CONCLUSIONS: The polymorphism of the CRP Gene is associated with occurrence of OSA and NAFLD in Asian Indian subjects. Asian Indian subjects carrying the allele SNP +1444 C>T (rs1130864)] polymorphism of CRP is predisposed to develop OSA and NAFLD. CLINICAL IMPLICATIONS: CPAP therapy decreases lipid profiles and hs-CRP levels in obese patients with OSA and NAFLD, OSA and without NAFLD and Without OSA and with NAFLD.
