Abstract
Introduction:. The incidence of re-admission of DM patients tends to increase because lack of education from health workers. One of the education that can be given to DM patients can be done in discharge planning. Objective: This research aims to examine in more depth the application of discharge planning to self-management and blood glucose levels in type 2 DM patients Method: The method for this review of literature used a narrative review approach. The data used is secondary data obtained from several databases such as Google Scholar, Pubmed, Science Direct, Proquest and Research Gate.The literature search in preparing articles used period of publication years from 2018 to 2023. Result: The results obtained from all the articles that have been reviewed show that discharge planning has an influence on the lives of diabetes mellitus patients after discharg from the hospital, both in terms of self-management, self-care, control of blood glucose levels. Self-management and blood glucose levels in diabetes mellitus sufferers are not only influenced by discharge planning, but can be influenced by other factors, one of which is providing diabetes self-management education (DSME). Conclusion:. Discharge planning has a positive effect on the health of patients suffering from diabetes mellitus after returning home from the hospital. This can minimize the possibility of complications that can be experienced by patients who suffers with diabetes mellitus.
