Abstract
In the version of this article initially published, there was an error in Fig. 7, where the decision points below “<6 months since cisplatin-containing definitive therapy?,” now reading left-toright as “No” and “Yes,” appeared originally as “Yes” and “No,” respectively. The errors have been corrected in the HTML and PDF versions of the article.
