Needle in a haystack - A case of functional eye convergence insufficiency

Abstract

Little is known about functional eye movement disorders (FEMDs). We present the case of a patient who was brought to the emergency department after an episode of unexplained loss of consciousness followed by horizontal diplopia in any gaze direction, and who was finally diagnosed with functional convergence insufficiency after meticulous work-up. The purpose of this case report is to recall a rare presentation of a FEMD to neurologists, psychiatrists and ophthalmologists. This case illustrates the importance of knowledge about FEMDs in the neurological clinic and of close interdisciplinary collaboration, in order to address the patient's problem as efficiently as possible without detours.
