Abstract

Se presenta el desarrollo de un cardiógrafo de impedancia para determinar el volumen minuto cardíaco y su correlación con el método de termodilución . Material y métodoPor medio de una fuente senoidal se inyectó una corriente de 50 KHz de frecuencia y de 1,5 a 3 mA de amplitud. Se realizó valoración de la repetitividad del método en sanos . En pacientes seleccionados, 31 en total, se colocó catéter de Swan-Ganz y se tomaron determinaciones en forma basal, posestimulación adrenérgica y betabloqueo . Se utilizaron las fórmulas de Kubicek y Srameck para la determinación del volumen minuto . ResultadosValoración de repetitividad : se observó una variación de hasta un 30% en el valor del volumen minuto en diferentes días de determinación . Comparación con el método de termodilución : a) con ecuación de Kubicek (17 pacientes) coeficientes de correlación en basales 0,72, posinotrópicos 0,92 y posbetabloqueantes 0,94; b) con fórmula de Srameck (14 pacientes) coeficientes de correlación basal de 0,17 y posinotrópico de 0,32 . Conclusión El método presenta muchas fuentes de error, por lo que es muy difícil su aplicación clínica actualmente, tanto en el seguimiento de los cambios del volumen minuto en valores absolutos como en el cálculo del porcentaje relativo de variación, aunque en este último punto es necesaria confirmación con mayor número de pacientes e investigando diferentes tipos de electrodosBackground We present the development of an impedance cardiograph for the cardiac output estimate and its correlation with the thermo dilution method.Material and method A constant electrical flow of 50 KHz frequency and of 1.5 to 3 mA amplitude is passed through a senoidal source. We carried out an appraisal of method recurrence in healthy people. A Swan-Ganz catheter was placed in a group of 31 patients and basal, inotropic and betablocker stimulation was performed. Kubiceck and Srameck formulas were used to estimate the cardiac output. Results Recurrence evaluation: a variation up to a 30%in the cardiac output was observed in different days of estimation. Comparison to the thermodilution method: a) with the Kubiceck equation (17patients) correlation coefficients in basals 0.72, after inotropics 0.92 and after betablockers 0.94; b) comparison with Srameck formula (14patients) basal correlation coefficients0.17and after inotropics 0.32. Conclusions The method has many sources of errors, therefore it's very difficult its clinical application now, both in the follow up of the cardiac output changes in absolute values and the relative variation in percentage count, but in this last point it's necessary the confirmation with more patients and differents electrodes types.
