Abstract

With an aging population, the demand for long-term care continues to grow. However, fragmented systems and inconsistent data hinder timely updates to care plans. This study presents an Intelligent Care App based on SMART on FHIR, using standardized data and clinical decision support to support dynamic care planning. The app adopts FHIR standards and incorporates CDS Hooks and HL7 Clinical Quality Language (CQL) to enable cross-system exchange and reduce caregiver workload. Initial testing suggests improved data flow and adaptive plan generation.
