Abstract
Glioblastoma is the most common malignant primary brain tumor. The US census divides the American population into "Hispanic/Latino" and "Not Hispanic/Latino", regardless of race. Hispanic/Latino is a rapidly growing population, but little is known about patterns of care, genomic profile and access to clinical trials in Hispanic patients diagnosed with glioblastoma. Retrospective review of glioblastoma in Hispanic patients evaluated at a tertiary referral center (University of Miami, FL). Analysis was restricted to patients seen after 06/2014, when routine molecular evaluation was incorporated into clinical care. A total of 54 self-declared Hispanic/Latino patients were identified. Among those, 48 were white, 6 from other races. Median age at diagnosis was 59 years. Twenty two (42%) patients had a gross total resection, 25 (47%) a subtotal resection and 7 (13%) biopsy only. Pathology review confirmed WHO grade IV GBM in all patients. Ten patients were diagnosed with lower grade glioma at initial diagnosis and had GBM at recurrence. IDH-1 R132H mutation was present in 14%. MGMT promoter was methylated in 27% of available samples. Next generation sequence (NGS) based assay was performed in 89% of the patients. The most commonly altered genes were EGFR, TP53, PTEN and TERT. Fifty three (98%) patients were treated with RT with temozolomide followed by temozolomide at diagnosis and a variety of treatment options at recurrence. Median overall survival for newly diagnosed GBM patients was 19 months. Ten (18%) patients were enrolled into clinical trials. In this series, Hispanic/Latino glioblastoma patients appear to have a similar genomic profiling as compared to general populations and achieved comparable survival and enrollment into clinical trials. This suggests that Hispanic/Latino populations with access to referral centers achieve outcomes comparable to the general population. Larger series and better racial reporting systems are needed to investigate the racial and genomic diversity within Hispanic/Latino population.
