(52) has been corrected below: a missing factor κ has now been included. This correction is necessary for consistency since the laser threshold correction scales with the cavity loss rate. 1 (Formula presented.) Two other typesetting errors have also been corrected: - An extra factor 2 has been removed from the expression of ξ− in the second column of page 11: 2 (Formula presented.) - The fifth sentence of the caption for Figure 2 has been corrected from “The cutoff for the Fock-basis is Ncutoff = 50 below threshold…” to “The cutoff for the Fock-basis is Ncutoff = 300 below threshold…”.
