Abstract

Aim: To estimate the economic burden of type 2 diabetes and its cardiovascular complications in Russia Federation. Methods: The analysis included direct costs and indirect economic losses due to type 2 diabetes (T2DM) and its cardiovascular complications (myocardial infarction (MI), stroke, ischemic heart disease (IHD)) in 2014. Data sources for analysis included federal statistics, Russian T2DM epidemiological studies, normative documents, Meta-analyzes, and local observational studies. Based on metaanalysis data and Russian epidemiology data we calculated the population attributable fraction of T2DM in cardiovascular diseases. Direct costs included hospitalizations, ambulance, outpatients care and medications both for T2D and its cardiovascular complications. Indirect costs included economic losses due to premature death and disability. Results: The population attributable fraction of T2DM in IHD morbidity was estimated as 7,45%, in IHD morality - 6,61%, in MI (both morbidity and mortality) - 2,83%, in ischemic stroke - 1,72% and in hemorrhagic stroke - 1,49%. Total economic burden associated with T2DM and its cardiovascular complications estimated as 569 billion rub/year, which is close to 1% of gross domestic product (GDP) of the Russian Federation (2014 GDP). Total burden consists of T2DM burden - 372 billion rub. and 197 billion. rub. burden of cardiovascular complications (IHD - 188 billion rub., MI - 7 billion rub., ischemic stroke - 7 billion rub., hemorrhagic stroke 2 billion rub.). The direct medical and nonmedical costs amounted to 142.1 billion rubles/year (25% of total burden). Indirect costs (losses in the economy), related to the shortfall in GDP due to disability, premature mortality, and due to temporary disability, is 426.7 billion rubles/year (75% of total burden). Conclusion: T2DM and it cardiovascular complications are associated with substantial economic burden. Potential opportunities for improvement of T2DM burden is in the investing in prevention of T2D and its cardiovascular complications as well as to the early diagnostics and early treatment of T2DM. Burden numbers can be the argument in the dialog with policy makers for investments in programs and actions towards diabetes prevention and control.
